A balanced scale: "Very satisfied / Satisfied / Neither satisfied nor dissatisfied / Dissatisfied / Very dissatisfied." Two positive, one neutral, two negative.

For sensitive topics, make negative answers even more accessible. "Have you ever experienced harassment at work? Yes, multiple times / Yes, once / No / Prefer not to say."

Including "prefer not to say" actually increases honest responses. Without it, people who don't want to admit an uncomfortable truth will often choose "no" just to move on. With it, they can avoid answering without lying.
